We are seeking a detail-oriented Medical Record Analyst to review, analyze, organize, and maintain patient medical records while ensuring accuracy, completeness, confidentiality, and compliance with healthcare regulations. The ideal candidate should have strong analytical skills, knowledge of medical terminology, and experience with Electronic Medical Records (EMR/EHR) systems.
Key Responsibilities
Medical Record Management
Review and analyze patient medical records for completeness and accuracy.
Organize, update, and maintain electronic and physical medical records.
Verify patient demographics, diagnoses, treatments, and clinical documentation.
Ensure proper indexing and filing of medical records.
Documentation Review
Audit medical records to identify missing or incomplete documentation.
Coordinate with physicians and healthcare staff to obtain required information.
Ensure documentation complies with healthcare regulations and organizational policies.
Maintain accurate documentation for patient care and billing purposes.
Data Analysis
Analyze clinical and administrative data to identify trends and discrepancies.
Prepare reports on record accuracy, documentation quality, and compliance.
Assist in improving documentation standards and workflows.
Support quality improvement initiatives through data analysis.
Compliance & Confidentiality
Ensure compliance with HIPAA and other healthcare regulations.
Maintain patient confidentiality and data security at all times.
Follow hospital policies for record retention and information management.
Support internal and external audits related to medical records.
EMR/EHR Management
Update and maintain patient records in Electronic Medical Record (EMR/EHR) systems.
Ensure accurate data entry and timely record updates.
Assist healthcare staff with EMR-related documentation issues.
Generate reports from EMR systems as required.
Collaboration
Coordinate with doctors, nurses, medical coders, billing teams, and administrative staff.
Respond to requests for medical records from authorized personnel.
Participate in training and quality improvement programs.
Stay updated with healthcare documentation standards and regulations.
Required Qualifications
Bachelor's degree in Life Sciences, Healthcare Administration, Nursing, Medical Records Management, Pharmacy, or a related field.
0–3 years of experience in medical records, healthcare documentation, medical coding, or healthcare administration.
Freshers with knowledge of medical terminology and healthcare documentation may also apply.
