We are seeking a detail-oriented Medical Record Analyst to review, analyze, organize, and maintain patient medical records. The ideal candidate should have good knowledge of medical terminology, healthcare documentation, data accuracy, and confidentiality standards. Key Responsibilities Medical Record Review Review patient medical records for completeness and accuracy. Analyze clinical documentation, physician notes, diagnoses, procedures, and treatment information. Identify missing, inconsistent, or incomplete documentation. Ensure medical records meet required documentation standards. Data Analysis & Documentation Extract relevant information from medical records accurately. Organize and maintain patient information in electronic health record systems. Verify patient demographics, diagnoses, procedures, and medical history. Maintain accurate records and update information when required. Medical Coding Support Assist coding teams by reviewing clinical documentation. Identify relevant diagnosis and procedure information. Required Qualifications Bachelor's degree or Diploma in Health Information Management, Medical Records, Life Sciences, Healthcare Administration, or a related field. 0–3 years of experience in medical records, healthcare documentation, medical coding, or healthcare BPO. Freshers with relevant healthcare qualifications may apply.
